EDD forms for adjusting previously filed returns


EDD Form DE 9ADJ,Quarterly Contribution and Wage Adjustment Form, is used to request corrections to a previously reported:  

  • Form DE 9, Quarterly Contribution Return and Report of Wages; and  
  • Form DE 9C, Quarterly Contribution Return and Report of Wages (Continuation).  

EDD Form DE 938, Voluntary Plan for Disability Insurance Quarterly Adjustment Form, is used to request corrections to a previously reported:  

  • Form DE 3D, Quarterly Contribution Return; and  
  • Form DE 9C, Quarterly Contribution Return and Report of Wages (Continuation).  

Revisions were made to the paper versions of the DE 9ADJ and DE 938 to make form completion faster and easier. (EDD Tax Branch News #510 (January 23, 2023)) The updated forms reduce the number of entries required to report name and Social Security number corrections. The instructions for both forms have also been updated.
